Service Providers: Make a Referral
Coming January 1, 2024
As of January 1, 2024, all funding and responsibility for access and centralized intake related to child and youth mental health will be transitioned from Contact Hamilton to Lynwood Charlton Centre (LCC).
Until that time, please reach out to Contact Hamilton at 905-570-8888 or info@contacthamilton.ca to receive further information or make a referral for child and youth mental health services.
What is the Access & System Navigation (ASN) referral process?
Service Providers are able to make a referral using the electronic referral form and send to ASN in the following ways:
- Fax (905) 389-8765, “Attention ASN.”
- Partner Portal (electronic submission) requires a username and password. If you do not have one, please contact cissupport@lynwoodcharlton.ca to make a request.

Is the process different for Coordinated Service Planning (CSP)?
Yes.
Coordinated service planning program assists children and youth with complex needs, and their families, manage and coordinate their multiple services.
Coordinated service planners help to develop a plan that helps service providers understand your goals for your child, youth and family.
